Care Wisconsin is a nonprofit care management organization specializing in the integration of health and long-term care services. For over 35 years, we have been filling gaps in community services and providing innovative solutions for seniors and adults with developmental and/or physical disabilities. We have roots as a human services agency. With the support of community volunteers, Care Wisconsin started the region's first adult day center in a church hall in Madison in 1976.
Today, we manage Family Care and Partnership, two of Wisconsin's public health and long-term care programs. These two programs serve more than 5,600 members across 22 counties in Wisconsin. In addition, we also offer Medicaid SSI, a health insurance program for individuals with a disability who qualify for Federal SSI, to more than 1,600 individuals.
Our mission and values:
To promote the quality of life of our communities by empowering others and working together to creatively solve unique health and long term care needs.
To operate on a sustainable financial basis through growth and continuous improvement.
Our culture is based on integrity, accountability and treating our members, partners and each other with dignity and respect.
